All student-athletes participating in football, will be required to complete a GCYFL Registration form.  Player eligibility is based on birthdate that the student-athlete will be on 7/31 of the upcoming season (not current age or last years age).  How old will your student-athlete this July 31st?

All participants in GCYFL Football and/or Cheer require an annual sports physical by a licensed medical professional.  This physical form must be stamped by the medical professional office in order to be accepted by the league.  If the medical office uses their own form, it must still have the same information and must indicate the participant is clear to play / participate in the sport (and must be stamped).

If a student-athlete is diagnosed with a concussion, the league will require medical clearance prior to allowing the student-athlete to return to play.  There is a specific RTP Protocol that must be followed prior to a medical professional clearing the player.

We encourage parents to use this form with your medical professional to ensure proper RTP Protocol is being followed.
